Maximo Forms: AI-Built Forms with Conditional Logic & Superior Analytics
Maximo Forms lets you build forms that collect responses straight into your workspace. Maximo can build, edit, manage, and fully control any form from chat, anyone can fill it at a public /f/{slug} link, and you get analytics that go beyond a simple response count — a view-to-submit funnel, per-question drop-off, answer breakdowns, completion time, and traffic sources. The Forms plugin is activated by typing @forms in the composer.
- Ask Maximo to build a form and it handles everything: forms_create once per form -> forms_add_field for each question while you watch the live builder -> optionally forms_set_logic for conditional questions and forms_update for the theme, submit button, and thank-you page -> forms_publish only when you're ready, which returns the public /f/{slug} link. forms_unpublish takes it offline; forms_close stops new responses while keeping the data.
- The Forms plugin is activated by typing @forms (or /forms) in the composer; its tools and guidelines load only while active, so an idle plugin costs zero tokens.
- Field types: short text, paragraph, email, phone, URL, number, date, time, single choice, multiple choice, dropdown, checkboxes, rating, linear scale, file upload, plus section and heading blocks for layout. Mark any question required and add help text or a placeholder.
- Conditional logic (Plus and above): show questions only when an earlier answer matches — for example reveal a 'Which plan?' question only when someone selects 'Yes, I'm interested'. Logic is enforced both in the respondent's browser and on the server.
- Superior analytics: views, starts, completion rate, average completion time, a submissions time-series, a per-question drop-off funnel, per-question answer breakdowns (bar and distribution), and top sources and devices. Free plans see the core totals and time-series; advanced sections unlock on Plus and above.
- Sharing: every form has a public /f/{slug} link with proper SEO, its own Open Graph title, description, and a dynamic OG image so it looks great when shared. You also get an embed snippet and a QR code, and can set an auto-close date or a hard response limit.
- Responses land in the Responses tab as a filterable table and export to CSV in one click. You can also map fields to your CRM and have each submission create a lead automatically.
- The respondent page is a clean, fast, mobile-first form with a liquid-glass shell and a solid, high-contrast card for legibility, in light or dark mode, themed by your accent colour. A honeypot quietly blocks spam bots.
- Limits are enforced by plan: Free gets 2 forms and 50 responses a month with basic analytics; Plus gets 15 forms, 1,000 responses, conditional logic, file uploads, custom themes, and advanced analytics; Pro gets 50 forms, 10,000 responses, and no MyTabulon branding; Max gets 200 forms and 50,000 responses a month.
- Forms follow the signed-in user's role. Owners, administrators, and members can build, edit, publish, and manage forms; viewers are read-only. Deleting a form removes all of its responses and analytics and is owner/administrator only — when Maximo deletes a form from chat it asks you to confirm first via Action Preview.
- Forms content is the user's: Maximo builds what you describe and refuses phishing, credential-harvesting, payment-data, or deceptive impersonation forms.
